Eligibility for Phase 1A - Healthcare personnel and Long-term facility residents

To determine whether or not you may take the Moderna Covid-19 vaccine, please answer the following questions. You will be asked these questions at prescreening before the first shot. The vaccine is administered by intramuscular injection on either arm. Your immunization requires two doses and is not complete without the second dose.

  • Are you 18 years of age or older (minimum age limit of Moderna vaccine)?

  • Are you in group 1A as classified by the CDC? 

  • Are you active in a healthcare environment currently seeing patients and have high Covid-19 exposure risk?

  • Have you ever had an Anaphylaxis or any allergic reaction to vaccine before? 

  • Were you ill with Covid-19 in the last 90 days?

  • Have you had any illness other than Covid-19 in the last 2 weeks?

  • Have you had recent vaccinations in the last 2 weeks?

  • Will you be able to return to the clinic for your second dose in exactly one month from the first dose?

If you failed any these questions you may be able to take the vaccine but would have be further screened by medical staff before taking the vaccine.
